Confidentiality
Whilst you are receiving sessions at Taylor Sports and Wellbeing, we will collect and record information about you and your sessions in the following ways. We record your contact details. This allows us to manage your sessions and keep you informed. If we become concerned about your well-being, we may inform your GP so that they can also be involved in your care. We will always aim to discuss this with you beforehand. If you provide us with emergency contact details in addition to your GP, we will record this information. Where appropriate, we may contact this person instead of, or in addition to, your GP. We will always aim to discuss this with you beforehand. Your counsellor will write a summary of what was discussed in each session. This means that your wellbeing practitioner has an ongoing record of your sessions, which allows them to provide continuity of care. To comply with GDPR, we must have a legal basis to record and use your data in this way. The legal basis we use is that you have given us your consent by signing this agreement. You have rights relating to your data, and these are listed separately. All information is securely held, either in locked cabinets or secure databases, for 7 years. We will not pass on your information to anyone outside the service, with the following exceptions. Where you, the client, request or permit information to be shared.Where the information you, the client, have disclosed is too serious for confidentiality to be maintained, including where your counsellor considers that you or others may be at risk of immediate harm.If we are required to by law – in the case of a crime being committed etc.
